Completed visa application form in English, signed by the applicant; for minors, signed by both parents or legal guardian.

Provide statements for the last 3 months from British personal accounts only, updated no more than 1 month before submission, showing stable transactions and a positive balance.

Provide a fully paid return travel ticket in English showing the applicant's full name as in the passport.

Provide a fully paid hotel reservation for the full stay in Slovakia confirmed by the Slovak hotel or pension, showing all guests' names, check-in and check-out dates, and the facility's name and address.

Official letter from the medical institution or doctor confirming your appointment or treatment and stating the length of treatment.

Medical insurance certificate covering the whole stay, valid throughout the Schengen Area, with at least EUR 30,000 coverage for urgent hospitalization or repatriation.

Valid signed passport with at least two free pages, issued within the last 10 years and valid at least 3 months beyond the requested visa.

Provide one copy of the applicant's passport bio data page.

One recent passport-size photograph meeting ICAO standards and not older than 3 months.

Provide fully paid return travel tickets in English showing the applicant's full name as in the passport.

UK eVisa proving immigration status is required; share code alone is not sufficient. It must be valid at least 1 month beyond departure from the Schengen area.
