Submit the typed and fully completed visa application form in Portuguese or English, signed twice and dated. Handwritten forms are not accepted.
- Medical institution confirmation
Document issued by the medical institution in Portugal confirming that you will receive medical care there, including the name of the responsible doctor.
- Proof of sufficient financial means to pay for the medical treatment
Provide pre-payment or other proof of sufficient financial means to cover the medical treatment and related expenses, such as insurance coverage.
- No objection certificate
Original employer NOC addressed to the Embassy of Portugal in Abu Dhabi, dated within 1 month, stating employment status, position, contract length, salary, approved leave dates, and purpose of visit.
